
SUPPORTIVE IN-HOME SERVICES
Serenity Alliance HealthCare, a Medicaid Waiver program incorporated in Virginia to provide services to individuals with developmental disabilities in their homes, supervised facilities (apartments) and/or choose service settings.
Serenity Alliance HC acknowledges that the individual is the most important person in his/her own development because he/she is most directly affected by his/her constraints and limitations, and as a program recipient. Individuals are empowered to make input into decision making about their lives and the services they receive to the fullest extent possible. Our commitment is further reinforced by the philosophy that we work for the individuals we serve. In addition to being a caregiving agency we also support individuals in improving their self-sufficiency, increase choice and gaining control of their life which intends improve their quality of life.
Admission Criteria

Management team and staff are pleased for the opportunity to provide services for the individuals referred by placing agencies. We do not discriminate on the basis of race, color, sex, national origin, mental or physical handicap, religion or ethnic background. In an effort to better serve the needs of individuals referred to us the following criteria MUST be met.

Individuals must be at least nine years of age.

Individuals must have a primary diagnosis of a developmental disability and may have a secondary diagnosis of mental illness.

Individuals must be tuberculosis free.

Individual’s service needs can be managed by Serenity Alliance HC.

The individuals must have significant limitations in adaptive skills in two or more of the following areas: communication, self-care, home living, social skills, community use, self-direction, health and safety, functional academics, leisure and work.

Only individuals eligible for DD waiver funding and funds are available.
