SUPPORTIVE IN-HOME SERVICES

Serenity Alliance HealthCare, a Medicaid Waiver program incorporated in Virginia to provide services to individuals with developmental disabilities in their homes, supervised facilities (apartments) and/or choose service settings.

It is the policy of Serenity Alliance HC to maintain a written mission statement, which guides staff behavior and articulates the goals and objectives of the corporation. Serenity Alliance HC is a Medicaid Waiver program incorporated in Virginia to provide services to individuals with developmental disabilities in the privacy of their homes, supervised facilities (apartments) and homes in natural settings.

Serenity Alliance HC acknowledges that the individual is the most important person in his/her own development because he/she is most directly affected by his/her constraints and limitations, and as a program recipient. Individuals are empowered to make input into decision making about their lives and the services they receive to the fullest extent possible. Our commitment is further reinforced by the philosophy that we work for the individuals we serve. In addition to being a caregiving agency we also support individuals in improving their self-sufficiency, increase choice and gaining control of their life which intends improve their quality of life.

Serenity Alliance HC recognizes that the individual is the most important resources in his/her own development, both as the person most directly affected by his/her constraints and limitations, and as recipient of the program. Our empowerment strategy therefore places control in the hands of the individual to claim both his/her rights and responsibilities to develop to his/her full potential. Our commitment is further reinforced by the philosophy that we work for the individual. Therefore, Serenity Alliance HC is not just a care giving facility; we exist to enhance the quality of life of the individuals we serve.

Admission Criteria

  1. Management team and staff are pleased for the opportunity to provide services for the individuals referred by placing agencies. We do not discriminate on the basis of race, color, sex, national origin, mental or physical handicap, religion or ethnic background. In an effort to better serve the needs of individuals referred to us the following criteria MUST be met.

    1. Individuals must be at least nine years of age.

    2. Individuals must have a primary diagnosis of a developmental disability and may have a secondary diagnosis of mental illness.

    3. Individuals must be tuberculosis free.

    4. Individual’s service needs can be managed by Serenity Alliance HC.

    5. The individuals must have significant limitations in adaptive skills in two or more of the following areas: communication, self-care, home living, social skills, community use, self-direction, health and safety, functional academics, leisure and work.

    6. Only individuals eligible for DD waiver funding and funds are available.

Populations Served

Serenity Alliance HC serves all individuals from the age of 9 with primary diagnosis of developmental disability who may have a secondary diagnosis of mental illness.

Our Services

All services are individualized and person-centered with the individual makes the decisions for his/her services and Direct Support Professionals deliver the services as chosen by the individual.
